Johns Hopkins Department of Medicine is seeking a Lifestyle and Health Coach to support participants in a research trial within the Mid-Atlantic Center for Cardiometabolic Health Equity (MACCHE) entitled: Understanding and addressing the risks of Low socioeconomic status And Diabetes for Heart Failure (UNLOAD-Heart Failure). UNLOAD-Heart Failure and MACCHE are led by the Johns Hopkins Center for Health Equity, which works to promote health equity for groups that have been socially and economically marginalized through advancing scientific knowledge, promoting sustainable changes in practice and policy, partnering with communities, raising public awareness of health inequities, and training scholars.


UNLOAD-Heart Failure is testing the effect of a multi-level community-based intervention, including problem solving training; partnership with community fitness facilities to promote healthy lifestyle; and support from community health workers, on outcomes in persons with low socioeconomic status, diabetes, obesity and early heart dysfunction.


The Lifestyle and Health Coach is a professional who provides coaching or counseling services for research participants. The lifestyle and health coach will facilitate the Problem-Solving Training and guide people on how to live a healthier, more balanced life using nutrition, lifestyle, and exercise.


Specific Duties & Responsibilities

  • Meet with participants (in person, virtually, or over the phone) to discuss goals, progress, and challenges.
  • Facilitate the bi-weekly Problem Solving Training and subsequent monthly training sessions.
  • Lead exercise sessions, and instruct patients in safe exercise procedures.
  • Develop and update care plans that are custom-tailored to individual participants.
  • Help participants identify areas for improvement in their personal health habits.
  • Provide guidelines and effective solutions to reduce or eliminate unhealthy behaviors.
  • Counsel and advise participants on how to improve their overall health condition.
  • Document and keep records of health concerns and progress toward goals.
  • Provide encouragement and support to empower participants.
  • Provide support and education about self-care practices such as diet, exercise, stress management, and coping skills.
  • Communicate with participants in a positive, empathetic and culturally sensitive manner to make them feel comfortable, motivated and respected.
  • Coordinate with the studies Community Health Workers in order to provide a well-rounded and supportive experience for study participants.
  • Travel to the YMCAs (33rd street and Druid Hill locations) and other community sites. Some weekend work may be necessary.

  • Possess a deep understanding of positive psychology, behavior change, and motivation.
  • Possess organizational and time management skills.
  • Ability to build strong, trusting relationships with participants.
  • Ability to resolve conflicts with participants and overcome their objections about their health and behaviors.
  • Ability to provide empathy in difficult interpersonal situations.
  • Eagerness to learn new skills, including problem solving training, exercise training and motivational interviewing.
  • Ability to travel to community-based exercise facilities to supervise and motivate exercise.


Physical Demands

  • Lifting and/or carrying objects weighing more than 50 pounds.
  • Pushing and/or pulling non-motorized equipment (e.g. stretchers, supply carts, etc.)
  • Walking and/or standing for extended periods of time.


Minimum Qualifications
  • Bachelor's Degree in Exercise Physiology, Health Science, Nutrition, or closely related field.
  • Minimum of 1 year of relevant experience.
  • Valid CPR and BLS certification required. ACLS certification preferred and will be provided if not already completed.
  • Valid Maryland driver's license and access to an insured automobile that lists employee on the insurance policy.


Preferred Qualifications
  • Education and/or work experience as an exercise physiologist. Must be knowledgeable about exercise in persons with chronic disease, such as diabetes and hypertension.
  • Training in coaching, fitness, or nutrition.
  • Experience working with people in a community setting, specifically with communities that have been socially and economically marginalized.

JHU Equivalency Formula: 30 undergraduate degree credits (semester hours) or 18 graduate degree credits may substitute for one year of experience.
