The Nurse Navigator of The Johns Hopkins Avon Foundation Breast Center will be responsible for enhancing awareness, education and outreach related to breast cancer. The Nurse Navigator is an essential link between patients, their physicians and other care providers. The goal of navigation is to ensure patients effectively and efficiently transition through the continuum of breast cancer care from newly diagnosed through surgical treatment receiving appropriate services, support and education. The patient will also be assisted in transition to other areas of care of the multidisciplinary team. This individual will function as an integral member of the Breast Center team, relating to patients, survivors, volunteers, faculty/providers, and outside health agencies related to breast cancer. Working closely with the Administrative Director, she will work to build on the already existent multidisciplinary programs devoted to prevention, diagnosis, care and treatment of patients at risk of or with breast cancer.
Specific Duties & Responsibilities
PATIENT/ FAMILY SUPPORT AND NAVIGATION
Provide patient navigator services
Serve as an essential link between patients and all other care providers
Follow patients throughout the care continuum from diagnosis through surgery
Determine support and resources available to each patient and family
Assess level of need for navigation and mutually plan with patient/family their navigation need
Connect patients with the Survivor Helping Survivors program with matching based on diagnosis, treatment and stage and various social needs (age, family, etc)
Facilitate consults and support services within established service standards including social workers, dietitians, physician, etc.
Partner with support programs and organizations (Red Devils, Hopewell, ACS, Komen) to provide services to patient/family.
Identify and refer specific pt/family needs to Social Work services.
Be a point of contact for patients who need follow up, questions answered or to make connections with other health providers.
Provide one-on-one patient education for newly diagnosed patients,
Assist patients in understanding their diagnosis, treatment options, and the resources available
Assess patient and family knowledge of diagnosis
Assess education level and past experience relating to current diagnosis.
Assess patient/family stress level impacting their understanding and decision making
Provide appropriate teaching, outreach and education to pt/family
Provide emotional support and resources
Assist patient and family in the transitions from diagnosis to treatment, treatment to survivorship
Address survivorship issues
Develop patient education programs and tools.
Develop an active patient resource data base using internet, periodicals, articles and books.
Johns Hopkins Breast Cancer Volunteer Program, comprised of cancer survivors and other volunteers.
Maintain data base of survivor volunteers to connect with patients
Contact the volunteer when a patient requests a survivor volunteer by phone and email. Keep record of connection and validate communication.
Maintain data base of all volunteers and their activities
Recruit new volunteers (through personal contact or self-referral)
Interview potential volunteers to assure ability, role and need
Orient new members to team and to Imaging quarterly
Promote and coordinate volunteer and survivor volunteer programs and materials, e.g. quarterly dinner meetings, announcements and news via email
Maintain and update the Volunteer and Imaging Volunteer training manuals
Connect new volunteers with experienced volunteer in buddy system of orientation and training.
Confirm documented hours of service to maintain active volunteers
Communicate with all volunteers on monthly basis via email newsletters
Plan agenda and promote quarterly dinner meetings
Establish a volunteer team executive committee
Assign volunteers and recruit volunteers to the community education and fundraising events
Manage the daily schedule of volunteers in Imaging
Maintain relationship and communications with Imaging techs who work with the volunteers
Promote individual volunteer involvement to a high level of commitment to fulfill the mission of patient support, advocacy, education and community outreach.
Strengthen and value the team effort and encourage leadership.
Support the volunteers whose disease progresses through stage IV and who remain active members through their death
Support the co survivor volunteers and help express their grief with understanding and compassion for all.
Collaborate with Volunteer Services following their rules and administrative requirements
Manage and Facilitate Breast Cancer Survivor and Metastatic Breast Cancer retreats.
Set goals and objectives of each retreat
Establish time line of activities and check list of responsibilities
Contact staff and communicate goals and objectives.
Request assistance, meet with each guest staff and confirm, send directions, follow up with thank you and feedback
Coordinate the volunteer staff and communicate expectations
Encourage the volunteers' feedback and leadership in parts of the retreats
Establish budget for spring retreat
Plan program or assist in design, goals, agenda, evaluation tools
Implement 3 to 4 retreats per year and act as coordinator
Facilitate the groups in discussion or co-facilitate the discussions in the weekend long retreat.
Prepare program materials
Establish an event planning guide for each retreat to streamline effort and use as educational tool for community programs
Prepare brochure, retreat description for internet and website
Notify staff and multidisciplinary team
Share brochure with regional breast and cancer centers and organizations
Maintain data base of contacts
Register participants after intake call or email
Prepare information for mailing of retreat details
Evaluate program at end and at 6 months
Revise program with feedback from evaluations, observation and input of volunteer staff.
BREAST HEALTH EDUCATION AND OUTREACH
Develop, plan and implement outreach programs for local community that encourages early detection, risk factors, healthy behaviors, and resources.
Participate in local health fairs
Develop the interactive health fair booth that engages each participant through questions and answer flashcards.
Maintain up to date educational materials
Utilize the volunteer team to assist with health education in their community, their unique population, or special interest.
Utilize volunteers in developing program
Collaborate, plan and coordinate breast cancer education programs for consumers and patients, and celebration events for survivors within the Hopkins community.
Participate in and coordinate fund-raising initiatives related to supporting activities of the Breast Center, particularly as it relates to funding for outreach and education programs.
Avon Walk team contact for JHMI
Coordinate volunteer role in other fundraising events with volunteer team captains and with volunteer membership (walks, rides, fundraisers)
Develop volunteer team role and educational role with PVDA Ride for Life
Collaborate with local and national breast cancer organizations that result in increasing breast health program compliance, and reducing mortality and morbidity through early detection and treatment.
Coordinate marketing activities for the Johns Hopkins Breast Center. This may include local, regional and national public speaking.
Assess audience, need and topic requested for population
Research topic for evidence based information
Prepare presentations (power point)
Participate in a variety of activities related to the Breast Center as assigned by the Administrative Director of the Breast Center.
Perform activities on behalf of the Administrative Director as needed in her absence.
Assist doctor or Nurse Practitioner with minor surgical procedures in clinic setting.
Supervision of Others: Yes, volunteer staff of 15+
Must be a registered nurse, licensed in the State of Maryland or state where practicing.
Bachelor's degree in nursing or related discipline required. Master's degree strongly preferred.
2 years' experience in the specialty or a related area required.
Additional experience may substitute for bachelor's degree. Related master's degree may be considered in lieu of experience
Current American Heart Association or American Red Cross CPR certification required. Must maintain current licensure and certification during duration of employment.
Acceptable completion of the JHH or BMC credentialing process is required within two weeks of start date.
Special Knowledge, Skills, and Abilities
Possible exposure to communicable diseases, hazardous materials, radiation, and pharmacologic agents, safety regulations must be followed.
Work schedules are determined by protocol activity and may demand flexible and/or extended work hours.

Maintain patient navigation certification.
